When my father continuously declined and needed help, we asked Gene (Professional Care Manager) to help us navigate towards his care. She found a place in Novato, met our father, and set up the care plan. She also coordinated my father’s transfer from the hospital to the Skilled Nursing Facility. Gene got us through the toughest times of our life. From coordinating the group of CNA/Caregivers that helped out my father, to working with hospice for my father’s care plan as he declined.
